- All exams and/or quizzes must be proctored by appropriate personnel unless otherwise specified by the instructor. The veracity of the proctor is determined by the apex office in coordination with the requirements of the instructor. Students testing at their place of employment typically have supervisors or human resources representatives serve as a proctor. Additionally students may have staff members at a local College or University serve as proctor. The proctor may not be enrolled in courses or degree programs at USF while serving.
- Testing sites/Proctors should be designated prior to the beginning of a semester. Students can designate a proctor from the drop down box within the registration page. If a student's proctor is not already lised within our database, they should scroll down to student tools area within the MyCOE account, click Edit Testing Site/Proctor Information, and then select option #3. All proctor information must be accurate and current. In the event of contact information changing, please notify the APEX office immediately at [813] 974.3783 or wfairman@admin.usf.edu. If a student does not designate a remote testing site/proctor he/she will be expected to come to campus to take the exam with the rest of the class.
- It is the student's responsibility to notify the proctors about upcoming tests at least 48 hours before the test, and more if possible.
- All exams/quizzes requiring a proctor must have the proctor present in the room for the duration of the exam.
- The proctor must ensure that all conditions outlined in the “Exam Cover Sheet” were met. The proctor’s signature certifies on behalf of the individual and the organization they represent that all conditions were met, that no cheating occurred, and that any and all anomalies were noted.
- Failure to ensure exam security may result in the disaffiliation of the represented organization and/or disciplinary action against the student.
- All exams, quizzes, or other secure course materials must be administered simultaneously with the on-campus administration. Alternate exam schedules must have prior written approval from the instructor. The documented approval must be forwarded to the proctor so that he/she is made aware of the new schedule. The proctor must attach a copy of the written approval to the completed exam prior to returning it to the apex office. Failure to take an exam at the designated time may result in exclusion from future apex offerings and/or academic penalties.
- Exams must be returned to the apex office immediately following the administration. Course materials are tracked by the apex office, and delayed receipt of exams may be considered suspicious circumstance by the instructor and therefore result in exclusion from future apex offerings and/or academic penalties including expulsion.
- Any and all anomalies during the exam should be noted by the proctor on the attached and signed cover sheet. Any suspicion of cheating on exams and quizzes will be considered by the instructor and possibly other College officials. Suspected cheating may result in exclusion from future apex offerings and/or academic penalties including expulsion.
- It is the responsibility of the student to notify their proctor of upcoming exams and quizzes in a timely manner. This way if scheduling conflicts occur, the instructor can be notified in due time and appropriate arrangements can be made.
- Exams and quizzes are sent to a student’s proctor one day before their administration except when instructors administer their exams on the Blackboard system.
- We would like to reiterate our diligence concerning sensitive materials and we trust our students have the same regard for exam security.

- The instructor will include with each exam/quiz an exam cover sheet outlining all requirements of the exam’s administration.
- Following the examination, the proctor must sign and date the Cover Sheet, attach it to the completed exam, and forward it to the apex office immediately.
- Exam materials must be returned to the apex office in the manner outlined by the instructor [overnight courier service, electronic scan sent via e-mail, or by fax followed by a mailed original]. Packaging should be marked confidential and treated as proprietary and secure correspondence.

- The apex materials coordinator is responsible for handling materials such as exams, homework assignments and term papers which are to be distributed to and collected from students viewing their classes at a remote location. Materials to be turned in should be e-mailed to wfairman@admin.usf.edu, faxed to the apex office at 813.974.8010, or shipped by FedEx, UPS or another similar delivery company that allows for in-transit tracking while en route to apex. - Course materials will be posted on the course page accessable through each student's MyCOE account. If you need to download the Adobe Acrobat Reader, a free copy can be found at http://www.adobe.com/products/acrobat/alternate.html. If necessary, materials may be e-mailed or shipped via UPS to the addresses specified by the student during the registration process.
- All incoming and outgoing materials are catalogued in our Materials Tracking database to be sure that all materials have been properly transmitted to their appropriate destinations. If a student has any questions about the delivery of a given material, he or she should contact the Materials Coordinator as soon as possible.
- A completed apex materials coversheet should be attached to all homework and projects sent back to apex.
